Becoming a patient
When your dentist determines that specialized treatment is required related to your gum health and the supporting structures of your teeth, they will provide a referral to our clinic. After receiving your referral, our team will contact you to schedule your intake appointment.
Before your appointment
Complete the medical history form. You may return the form by email or print it and bring it to your appointment.
Helpful tip: After clicking the download button below, save the PDF to your device. You will then be able to type in the PDF. Be sure to save the PDF after completing it and then you can email it to our office.
If you cannot complete the form online, you may complete the form in person when you arrive. Please arrive 15 minutes before your scheduled appointment time to complete the form.
